Pathways Home East Tampa
Creating a Way Forward
A coordinated approach connecting neighbors experiencing homelessness with outreach, services, and pathways toward stability — one relationship, one step, one neighbor at a time.
Homelessness is rarely solved by a single service. A neighbor experiencing homelessness often needs many things at once — identification, health care, a safe place to stay, transportation, and someone who will walk with them. Pathways Home East Tampa is FAMA's coordinated approach to connecting people with the services and relationships that move them toward stability.
FAMA works alongside partner organizations across the continuum below. We help coordinate the path; we do not imply that FAMA directly provides every service, nor do we guarantee housing outcomes. Instead, we connect neighbors with the partners and resources best equipped to help at each step.
The Continuum
From Outreach to Reintegration
Street Outreach
Meeting neighbors where they are and building trust.
Health & Mental Health
Connecting people with the care they need.
Identification & Benefits
Helping restore the documents and benefits that open doors.
Transportation
Removing the distance between a person and the help they need.
Shelter & Treatment Referrals
Connecting to safe shelter and treatment pathways.
Employment & Life Skills
Building the skills and income that support stability.
Community Reintegration
Reconnecting neighbors to relationships and belonging.
Each step builds on the last — moving a neighbor from survival toward stability.
Help Create a Way Forward
Your support helps FAMA coordinate outreach, connections, and care for neighbors experiencing homelessness in East Tampa.
Pathways Home–East Tampa is a place-based, low-barrier, relationship-centered approach to street outreach. Grant Park is the primary focus, with Highland Pines, Sulphur Springs, and the broader East Tampa area informing partner development.
A trusted outreach relationship helps a neighbor reach the next step: healthcare, identification and benefits, shelter or treatment referrals, housing connections, employment, and renewed community relationships. We work with specialized providers and follow up when a referral encounters a barrier.
The $500,000 founding campaign seeks to build the capacity behind that pathway: outreach staffing, transportation, coordinated follow-up, and practical service resources.
Join the founding partnership conversation on November 12, 2026, with networking at 8:30 AM and the meeting from 9:00–11:00 AM Eastern. Ask about partnership, referrals, volunteering, or a founding gift.
