A place-based, low-barrier, relationship-centered approach to street outreach — connecting neighbors experiencing homelessness with the care, healthcare, and pathways that lead to stability.
Homelessness is rarely solved by a single service. A neighbor experiencing homelessness often needs many things at once: identification, healthcare, a safe place to stay, transportation, and someone who will walk with them consistently.
Pathways Home East Tampa coordinates across seven steps: Street Outreach → Health & Mental Health → Identification & Benefits → Transportation → Shelter & Treatment Referrals → Employment & Life Skills → Community Reintegration.
